About Balance & Vertigo

Your sense of balance depends on the inner ear, the brain, the eyes and the muscles and joints working together. The inner ear contains tiny canals and sensors that tell your brain how your head is moving. When this system is disturbed, you may feel the room spinning, lightheadedness, unsteadiness while walking, nausea, ringing in the ears, or a feeling of fullness in the ear.

At Centosa, our ENT specialists and audiologists look for the cause of vertigo, dizziness and balance problems. We use a detailed history, ear examination, hearing tests and balance assessments to reach an accurate diagnosis. We then offer personalized treatment, from positioning maneuvers for ear crystal problems and medicines to balance exercises and, when needed, further care for inner ear conditions.

Steady steps and a calm head. The right diagnosis helps the world stop spinning.

Frequently Asked Questions

Find answers to common questions about vertigo, dizziness and balance problems.

When should I see an ENT doctor for dizziness or vertigo?
See a doctor if you have spinning or room-moving sensations, unsteadiness, frequent dizzy spells, nausea or vomiting with dizziness, or dizziness along with ringing, ear fullness or hearing changes. Also get checked if you have had a fall or feel afraid of falling. Seek emergency care immediately if dizziness comes with sudden weakness or numbness, slurred speech, double vision, severe headache, chest pain, fainting, difficulty walking, or after a head injury.
What causes vertigo and balance problems?
Most vertigo comes from the inner ear, which controls balance. The most common cause is BPPV (benign paroxysmal positional vertigo), where tiny crystals move into the wrong part of the inner ear and trigger short spinning spells when you turn your head or change position. Other causes include vestibular neuritis or labyrinthitis (inner ear or nerve inflammation, often after a viral illness), Meniere's disease, vestibular migraine, ear infections, head injury and some medicines. Less often, vertigo is caused by problems in the brain, which is why proper evaluation matters.
How is vertigo diagnosed?
Diagnosis begins with a detailed history of your symptoms, such as how long spells last and what triggers them, followed by an ear examination. Bedside tests such as the Dix-Hallpike manoeuvre and the head impulse test check eye movements and inner ear function. Depending on your symptoms, you may also need a hearing test, videonystagmography (VNG) or balance testing. An MRI scan is advised only when a brain-related cause is suspected. These tests are generally painless.
Can vertigo be treated or cured?
Yes, most causes of vertigo can be treated. BPPV is often relieved within one or a few sessions with a simple head-positioning manoeuvre such as the Epley manoeuvre, done in the clinic. Inflammation of the inner ear is managed with short-term medicines and vestibular rehabilitation exercises that help the brain adapt. Meniere's disease and vestibular migraine are controlled with a combination of medicines, diet and lifestyle changes. Treatment depends on the cause, so a proper diagnosis comes first.
What can I do at home during a dizzy spell, and how do I prevent falls?
During an episode, sit or lie down in a safe place, keep your eyes on a fixed point and avoid sudden head movements. Do not drive or climb ladders until you have been assessed. Drink enough water, avoid very long gaps between meals, and get regular sleep. At home, keep floors clear, use good lighting and non-slip mats, and consider handrails in the bathroom. Do not take dizziness medicines for long periods without medical advice, as they can slow recovery. Your doctor or therapist can guide you to balance exercises that suit your condition.
